EndoCowboy® File Retrieval
Fractured instruments in root canals pose a significant endodontic challenge, particularly when multiple fragments are involved. This case presented as a highly complex retreatment of tooth 37, with radiographic and clinical evidence of at least two separated files within the root canal system.
One fragment was tightly lodged in the curved section of the mesial buccal canal, complicating its retrieval. After extensive preparation, the removal was finally carried out with repeated use of the EndoCowboy system. The entire procedure required more than one hour due to the curvature and the length of the fragment, which made the removal challenging.
This case highlights the importance of srategic canal preparation, flexible retrieval systems, and patience when managing severely curved canals with retained instruments.
Pre-OP X-Ray
Pre-operative X-ray showing at least one separated file in the mesial buccal root canal of tooth 37.
Visible File
After one hour of preparation, the fragmented file finally became visible under the dental microscope.
2nd File
During the preparation the second file in the mesial lingual root canal became visible.
Post-OP X-Ray
X-ray demonstrating the retrieval of two fragmented files. Unfortunately there is more, in the canal behind.
